Living Life My Way

Living My Life My Way provides the experience, skills and resources to support people with disability to transition to supported independent living of their choice.

We work with people with disability, their families and service providers to achieve great outcomes and a secure future. This is a unique program in Gippsland which fully supports people with disability and their families during all stages of their transition.

The Living My Life My Way program is for people with disability who live in shared supported accommodation, or in their family home, who would like to change the way they live.

Our Living My Life My Way coordinator will coordinate the support and resources they need to achieve their goal, including:

  • Assessing the current level of living skills
  • Identifying and matching potential housemates
  • Supporting the search for appropriate accommodation
  • Strengthening support circles with family and friends
  • Developing peer support networks
  • Assisting in the process to transition to independent living funding packages
  • Providing on-going monitoring and evaluation to ensure that the person’s needs are being met once they have moved to independent living
  • Reviewing the person’s support needs and linking them with support and resources as appropriate

Who can use our Living My Life My Way program?

Anyone can use our Living My Life My Way services. You may already receive service funding from the Department of Human Services.

If not, you may be eligible for this funding. To find out if you or the person you care for is eligible for funded services, you will need to contact the Department of Human Services.
